Services

Homemaker Services provide needed assistance to individuals to complete activities of daily living that they are unable to perform alone. Homemaker Services may prevent or delay premature or inappropriate institutionalization, making it possible for individuals to continue living in their own homes or to return to their homes.  

Covered Services

Homemaker Services are day to day household tasks including:

• Housekeeping (dusting, cleaning, vacuuming)
• Laundry
• Grocery shopping
• Meal preparation  

Service providers - An individual who qualifies will receive Homemaker Services as approved in the individual's care plan.

Personal Care Services provide needed assistance to individuals who are unable to complete personal care tasks alone. Personal Care Services may prevent or delay premature or inappropriate institutionalization, making it possible for individuals to continue living in their own homes or to return to their homes.

Covered Services 

Personal Care Services may include assistance with the following tasks:

• Personal hygiene

• Bathing

• Washing hair

• Dressing

• Grooming (i.e., combing hair and shaving)

• Eating

• Toilet Use

Service providers - An individual who qualifies will receive Personal Care Services as approved in the individual's care plan. 

Nursing Services provide needed assistance to individuals who have physical and/or chronic health conditions which limit their capability to function independently. Nursing Services may prevent or delay premature or inappropriate institutionalization, making it possible for individuals to continue living in their own homes or to return to their homes. 

Covered Services

Covered services Nursing Services include the periodic evaluation, counseling, and/or screening to promote and maintain an individual's optimal health status and includes the following tasks:

• Conducting physical assessment and monitoring health status 
• Consulting with pharmacist and/or physician
• Monitoring, setting up and dispensing medications
• Giving injections
• Drawing blood
• Changing wound care dressing
• Providing foot care and nail care
• Providing nutritional assessments
• Providing health education

Service providers - An individual who qualifies will receive Nursing Services as approved in the individual's care plan.
